To get to your patient portal, you’ll need to contact your medical provider’s office for the web address. They can also provide you with instructions on how to register and log in if you need them. To enroll in the Patient Access Portal, obtain a unique access code from the Medical Records department, or give the registrar your personal email address when registering for your visit and your unique access code will be emailed to you. Once you have that code, use the links below to either complete your registration or login to the portal.
