The myCare Patient Portal is a secure online website that provides our patients with convenient 24-hour access to personal health information including: medical history, medication lists, discharge instructions, test results, educational materials and non-urgent communications with participating WMCHealth providers.

How do I sign up for a portal for Doctor’s visits?

After providing your basic information, you will receive an email with an activation link. Follow the prompts to complete the sign-up process. To sign up for a portal for your doctor office visits, call your Ascension Medical Group doctor’s office or simply talk to them when you are there for a visit.

What to do if you have issues logging into your patient portal?

If you’re having issues logging into your patient portal, please contact your medical provider’s office for support.

Can you use a portal locator on a patient portal?

Yes. Using the portal locator doesn’t affect your ability to use other URLs to log in to your patient portal.

How to Get an Activation Code

If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility, visit MyChart to get an activation code. Once you have your activation code, you can setup your username and password on the MyChart website .

Login to MyChart

To login to MyChart, visit mychart.baptisthealth.com. Once on the page, enter your username and password that you setup when your created your account and click Sign In.

How to Use MyChart

After logging into your MyChart account, hover over the Messaging tab and select “Ask a Question”
