How to set up a proxy account?

The first step in setting up a proxy account is to make sure both you and the other person have a MyChart account. To link your child’s account, ask the front office staff the next time you visit your pediatrician’s office. You also can do this for another adult whose account you wish to link to yours.

How do I get started?

Please speak with the receptionist when you check out at your next visit and she will be happy to assist you with registering for the patient portal. You will receive an email from AnMed Health containing a link. Click the link and follow the instructions to set up your account.

To contact a hospital chaplain, ask your nurse or the hospital operator to page 3636 for the on-call chaplain at AnMed Health Medical Center or 3737 for the on-call chaplain at AnMed Health Women’s and Children’s Hospital.
