Telephone Consult (TCON)

Telephone consults are requests that can be completed without an appointment. These are not be confused with virtual appointments. Providers review TCON and determine the best action needed to grant the request.

Annual Periodic Health Assessment

Annual periodic health assessments (PHA) are completed by different clinics for each service.

Additional Information

To initiate a medical clearance for retraining, PCS clearance for active duty personnel, special duties, profiles, such as 469 or 422, security clearances and PME 422s.

My Penn State Health

A patient will use this portal, if you have been hospitalized at Hampden Medical Center, Milton S. Hershey Medical Center or St. Joseph Medical Center.
