What format is the Alpine Medical Group form in?

All of our forms are in pdf format, which you can read using Adobe Reader . If you are a returning patient, save time by filling out this form ahead of time and bring it in with you for your appointment. This form allows Alpine Medical Group to release and/or receive protected health information. Completion of this form is voluntary.

How do I sign up for the patient portal?

Our patients have access to their own labs (drawn here at the clinic) through the patient portal. To sign up for the patient portal, please call our receptionists at 801-407-3000 to be assigned a user name and password. If you receive an email from the helo app instructing you to access the patient portal from there, ignore those instructions.
