School Forms

ALL Pediatrics has copies of the Virginia, Maryland, DC and Virginia Sports physical forms in our office. Our providers complete and provide only the physical exam section of the form. If you have a special form, please bring it to the office for completion.

Pre-visit Developmental Screening Forms

Please return the completed forms to our office one week prior to your visit. This will give our staff time to score and assess the results so that your child’s provider can address the results with you during your visit.

What is urgent care scheduling widget?

The urgent care scheduling widget is intended for acute care (fever, cough, ear pain, sore throat, etc.,) only. The urgent care online booking widget should not be used to schedule appointments for check-ups, sports physicals, behavior concerns, ADHD, depression, or any chronic conditions which may require additional time to address appropriately.

What to bring to a dental appointment?

IMPORTANT: Please bring your Photo ID and your Insurance Card to your appointment. If you do not bring your insurance card, payment will be due at the time of service.

The Valley Hospital Patient Portal

Retrieve Valley Hospital-related medical information such as lab results, diagnostic imaging results, medications received while in the hospital, and a record of appointments for hospital services. You'll be asked at the time of your registration if you would like to sign up.
