Patient Forms

You may download, complete and bring the form to your appointment. If you wish to submit the information online, please send us a request with a working email address. We will then send you log-in information for the patient online portal/kiosh.

Fees

We will verify the specific requirements in your insurance contract regarding deductibles, payments, copayments, and the terms regarding referral numbers, number of allowable visits to specialist, and in-office procedures. If your insurance requires you to pay a co-pay, then you must pay this co-pay at the time of your appointment.

Vitreoretinal and Macular Surgeries

Dr. Mai is a vitreoretinal surgeon who has extensive training and experience in advanced retinal surgical techniques. She maintains surgical privileges at multiple hospitals and surgery centers in the Arlington – Fort Worth – Alliance Keller area.

Office Procedures

Most procedures in the office are performed using topical anesthesia, usually eye drops. Timing range from a few minutes with intravitreal injections to half an hour with retina photocoagulation (laser). Patients usually can return to work the next day.
