Appointments can be made online or by phone Monday through Friday beginning at 8am.

2. Print and Complete Your forms

Save time by completing your forms before your visit. Please visit our patient portal and download the New Patient Packet, as well as any additional forms required by the physician you are scheduled to see.

3. Prepare for your office visit

The more we know about your condition and symptoms, the better care we can provide. Please click the button below to prepare for your visit.

Please remember to bring your insurance card and photo identification, and arrive 20 minutes early in order to complete any additional paperwork before seeing your physician. Please present your completed forms to the receptionist upon your arrival.

Accepted Insurances and Related Information

Please bring your insurance card with you to each visit, and inform us if there has been any change in your coverage or in your address. As changes may occur int he plans that we accept, please verify our acceptance at the time you make an appointment.

Self-Pay Patients

If you are a self-pay patient or if we are not able to verify your insurance at the time of registration, we will require a minimum fee of $250.00 towards your anticipated services. Additional fees may be assessed at the time of check-out.

Benefits and Eligibility

All Florida Orthopaedic Associates will make every effort to verify your coverage prior to your appointment, yet we cannot guarantee coverage of services by your insurance company.

What is a referral form for insurance?

Referral form if your insurance plan requires it. All medications, vitamins and herbal supplements or a list of these items, including dosage and frequency. X-Rays and MRIs you’ve had taken that may pertain to your problem. Medical records from the emergency room or other physicians that may relate to your problem.

How long does it take to refill a prescription?

We routinely check for messages throughout the day and will process your request as soon as possible. Prescription refills can take 24-48 hours to process.
