How long does it take to get the results of Alivio?

Can I get a COVID-19 test at Alivio Medical Center?

Alivio patients and non-patients are eligible for testing. Testing will be done by reservation only at the following Alivio clinics:#N#(Please note hours may vary dependent on community happenings)

Do I need an appointment to get a COVID-19 test?

Yes, please call 773-254-1400 to schedule your reservation. An Alivio representative will assign you a time slot for your test.

Where should I go for my test?

You can locate one of our greeters and they will be able to assist you. If not in a vehicle, you may also go to the main entrance and the screeners at the door will direct you. Please continue abiding by appropriate social distancing and face-covering guidelines as you wait for assistance and testing.

What is the cost of a COVID-19 test?

There is no cost for uninsured patients or for insured patients who are in-network with an Alivio provider. If you receive a bill, please contact us immediately so we can appeal or waive the charge. Patients who are out-of-network may still get a test at Alivio, but they may receive a bill for these services.

Do I need to bring any documents with me for a test?

Patients should bring their photo ID (driver's license/state ID, passport, matricula) and insurance card if they have one. If uninsured, proof of income is required. When you make your reservation, our patient service representatives will instruct you on the appropriate documentation to bring.

What is the COVID-19 test like?

When you call to make an appointment, you'll be asked some basic screening questions to make sure that testing is appropriate. Once you have arrived to your confirmed appointment, please remain in your vehicle while we perform an assessment and complete the test.

What happens after my COVID-19 test?

Alivio will contact you with your results in about 3-5 days. While you wait for your results, please continue to isolate yourself from others to minimize the risk of getting other people sick. If you are negative, we ask that you continue practicing social distancing, frequent hand-washing and other measures to prevent future infection.

Diabetes Self-Management Education

The program is designed to empower our patients in efforts to achieve optimal health-related outcomes. We offer comprehensive, multidisciplinary diabetes education for pre-diabetes, Type 1, Type 2 and Gestational Diabetes. Individual sessions are tailored to meet specific needs.

Weight Management Program

Our bilingual, registered dietitian/nutritionist has one-to-one sessions in underweight, overweight and obesity for adults, adolescents and children. The children and families are encouraged to attend our nutrition and physical activity class every first Wednesday of the month at the Western Avenue location.
