A patient and consumer health portal that saves patients time and offers the convenience of accessing their health information from Delta Health 24-7 is now available. Patients will need to have a valid email address on file at the hospital in order to access the patient portal.
The Patient and Consumer Health Portal allows patients and their families to see data from both acute and ambulatory visits to the Hospital. Patients can review visit details and update information. Users also benefit from access to preventative care information and immunizations.
DC Health Search helps residents and visitors find primary care facilities in the District of Columbia. Users can enter a address to search for nearby primary care centers within a mile or up to 10 miles.

Integrated features include: Updates to demographic or contact information made in the Profile area of the Portal’s Health Information Management.
Patients and authorized family members may access a Health Summary from the Portal, or transmit the Health Summary which serves as a Continuity of Care Document (CCD). Patients may safely transmit an encrypted summary to a specific physician by entering a Provider Direct Address.

Annegret Schmitt-Johnson is a board-certified Family Nurse Practitioner and joined Alder Brook Family Health in 2014. Annegret grew up in Germany and completed a degree in medical lab sciences. After moving to Vermont in 2002 she completed a Master of Arts in literature as well as a Master of Science in Nursing from the University of Vermont.
Diane Collias, LICSW, is a Licensed Independent Clinical Social Worker. She received her Master of Social Work from Loyola University-Chicago in 2009. Diane began her career in medical social work with a diverse patient population, working in outpatient oncology at University of Illinois at Chicago Medical Center.
Dr. James Dougherty is Board Certified in Family Medicine and has special interests in sports medicine and preventive care. He was a Chief Resident at the University of Vermont’s Family Practice Residency Program until joining Evergreen Family Health in 1998. Originally from Illinois, he resides in Underhill with his wife and two daughters.
Diane C. Rippa M.D. is a Board Certified Family Medicine physician with special interests in Women’s Health and Geriatrics. She grew up in Burlington and graduated cum laude from Harvard University.
Melisa Gibson joined AlderBrook Family Health in October 2021 after working at Milton Family Practice for 14 years. She grew up in Minnesota and went to college in Michigan. She went to Rochester School of Medicine, and completed her Family Medicine Residency at UVM.
