Affinity Health Center's Main Site is located at 455 Lakeshore Parkway, Rock Hill, SC 29730 in Tech Park South, near I-77 exits 79 or 77.

Additional Locations

Our clinics are located throughout York County in Clover, Fort Mill, and York. These health center sites have a Clinical Provider on-site to provide primary care from 8 am - 5 pm, days of operation vary by location.* Lab services are available at the Clover location. Appointments 803-909-6363.
