Come to the HIM (Health Information Management) office located in Hanford, Selma, Reedley, Tulare or the Adventist Health Medical Office (clinic) that you need records from. Please see location and hours of operation listed below. Requests must be submitted to the Health Information Management Department (Medical Records).
All charges for medical records will be billed to the requestor. Please contact the Health Information Management (HIM) department for your facility by calling the number listed under Records Request Forms and Contact Information or by visiting the campus HIM office for any medical record needs.
Patients over the age of 18, or a legal guardian, can easily view and download information from their electronic health record at no charge by creating a patient account online. For some hospitals or physician offices, your online account will also give you access to secure patient-to-doctor messaging.

AMITA Health does not provide copies of birth or death certificates. For information about how to obtain a copy of these certificates, contact your county clerk.

There might be a charge for medical records if being a request by a patient or patient representative. If the records are needed for continued care, there is no charge to the patient when records are sent directly to your physician or the facility providing treatment.
A form must be completed for each physician to whom you need your medical records mailed or faxed.
