How do I get my medical records from Adventist Health?

Come to the HIM (Health Information Management) office located in Hanford, Selma, Reedley, Tulare or the Adventist Health Medical Office (clinic) that you need records from. Please see location and hours of operation listed below. Requests must be submitted to the Health Information Management Department (Medical Records).

How do I request medical records for my facility?

All charges for medical records will be billed to the requestor. Please contact the Health Information Management (HIM) department for your facility by calling the number listed under Records Request Forms and Contact Information or by visiting the campus HIM office for any medical record needs.

How can a patient access their electronic health record?

Patients over the age of 18, or a legal guardian, can easily view and download information from their electronic health record at no charge by creating a patient account online. For some hospitals or physician offices, your online account will also give you access to secure patient-to-doctor messaging.

How to pick up medical records in hospital?

If you would prefer to make arrangements to pick up your records in person, call the hospital's Health Information Management department. For security reasons, you will be asked to provide a state or federally issued photo I.D.

What age do you have to be to get a medical record?

State law requires patients age 12 and older to authorize the release of records containing highly confidential information, including behavioral health, substance abuse, pregnancy, HIV/AIDS and sexually transmitted diseases. A minor who is emancipated, pregnant or has a child.

Does Amita Health provide birth certificates?

AMITA Health does not provide copies of birth or death certificates. For information about how to obtain a copy of these certificates, contact your county clerk.

Do you need a photo ID to pick up a patient's medical records?

For security reasons, you will be asked to provide a state or federally issued photo ID to pick up records. Unless arrangements are made in advance, we will not release records to individuals other than the patient or their legally authorized representative.

Why create an online patient account?

Create an online patient account for a safe and simple way to access information from your electronic health record (EHR). For privacy and security reasons, some medical records will not be available online.

Do you charge for medical records?

There might be a charge for medical records if being a request by a patient or patient representative. If the records are needed for continued care, there is no charge to the patient when records are sent directly to your physician or the facility providing treatment.

Do you need to complete a medical record form?

A form must be completed for each physician to whom you need your medical records mailed or faxed.
