Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call 804-320-DOCS during regular business hours.
New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Please contact our receptionist during office hours at 480.969.4138, or schedule your follow-up appointment as you leave our office. If you must cancel an appointment, please call at least 24 hours in advance, so that we may accommodate another patient.
The following forms can be downloaded to your computer and printed at home. Our Surgeon’s request all New Patient Paperwork be completed prior to your appointment. This allows them a chance to review your information and make their best surgical recommendation.
It is extremely important that all your pre-op testing is completed 2 weeks to a minimum of 1 week prior to surgery. Non-compliance may result in the postponement of surgery. Do NOT eat or drink anything for 8 hours prior to your surgery including gum, water, and candy.
Diet – You may eat whenever you like following a procedure but most patients find a bland or liquid diet to be preferable for a day or two.
