The completed form can be mailed, faxed or dropped off to the Health Information Management (HIM) Department, located at Adirondack Medical Center in Saranac Lake. You will need a form of ID to pick up medical records (example: driver’s license) On the authorization form, you can indicate where you want your records sent.

The Health Information Exchange is different from the Health Insurance Exchanges that are being set up as part of the Affordable Care Act.
Adirondack Health utilizes an Electronic Medical Record (EMR) program so that your health-care provider has access to your health history in a timely and efficient manner. A comprehensive EMR allows medical professionals to access your health information from a secure source throughout the Adirondack Health system.
