As part of HIXNY, Adirondack Health uses a patient portal to give patients access online to their medical records.The patient portal facilitates better communication with your physician's office by providing secure and convenient access from the comfort and privacy of your own home or office.
A. In order to have a patient portal account you will need to already be a patient at this practice. To activate your account, you will need to receive an activation code from us. Once you have received the code, you will be able to create your own username, password, and other login information used to verify your identity.
Having a patient portal account helps the practice create a more accurate record of your medical history as well as educate you on certain aspects of your health. It provides you and the practice with a more efficient means of communication, eliminating sources of frustration such as phone tag.
Once you are logged into your portal account, the information you view pulls from your own personal medical record. Only you will have access to your health records, including any messages or information sent to you from the practice.
Established patients may also contact our patient portal helpline at 518-824-8620 Monday through Friday between 9 a.m. and 5 p.m. for assistance in registering for the Patient Portal.
Any active patient may be eligible to register for and use the Patient Portal. If you are authorized, a family access account can be created that will allow you to access selected family members’ health information.
