Your health information is available for your personal records or continuing medical care with other providers. You may find some information in your Patient Portal, but if it is not there or you are not set up for the portal, copies of your results or notes may be requested from our Health Information Management Department (HIM).
We understand that you may have family or friends involved in your health care. For anyone to access your record, you have to give permission every time. This includes your spouse, adult child, parents of 18 and older patients.
