Call for Your Appointment

Please call (479) 444-1440 to schedule an appointment. If we do not have your information, or if your information has changed, please notify us at this time and have your insurance information available. If there is a particular time or day of the week that is convenient for you, please let us know.

Your First Visit

Normally, we ask you to arrive 30 minutes prior to your appointment time in order to complete the necessary paperwork. However, if you have completed the Patient Registration Forms in advance, please arrive 15 minutes prior to your appointment time.

Your Appointment Day

Please bring your current insurance card to your appointment. If you have already provided your insurance card at a previous appointment and we have scanned a copy into our system, this is not necessary unless the information has changed.
