At this time, your existing Patient Portals ( MyLink/My Doctor and MyLink/My Hospital) will no longer be updated; but they will be available for you to review or download your historical medical information. WHAT TO BRING

Valid insurance card, and photo ID. Should your insurance company require a referral please call your primary physician to obtain a referral before your appointment. If we do not have a referral your visit will be rescheduled.

CO-PAYS

Co-pays are due at the time of service. We accept cash, check or credit cards. We do not bill for copayments.

LATE ARRIVALS

Please arrive 15 minutes before your appointment. If you arrive after your appointment time it is possible your appointment might be rescheduled.

CANCELLATION AND NO SHOW POLICY

We ask that you call the office 24 hours before your appointment if you need to cancel or reschedule. If you don’t cancel within the 24 hour notice or do not show up for your appointment a $40 fee will be charged.
