Everyone must have an appointmentbefore entering the Yale Health Center (except for Pharmacy and Acute Care visits). Visitors are not allowed at this time. One parent may accompany their child to their appointment. Everyone entering the Yale Health Center is required to wear a mask, even if you are vaccinated.
MyChart is an online service offered by Yale New Haven Health that allows you to connect with your doctor's office, schedule appointments, request prescription renewals, review your health history and more — any time! All you need for access is an activation code from your doctor and your date of birth.
When you launch your internet browser, it automatically takes you to the Yale New Haven Health System page. To get to your delivery network's home page, click the dropdown menu in the blue “Yale New Haven Health” bar in the upper left, and choose your delivery network from the list.
To reactivate your account, please contact the MyChart Customer Service Center 8 a.m. to 4:30 p.m. weekdays: submit an online form. After verifying your identity, we will give you a temporary password to use to login; you'll then be asked to create a new password.
Click on the "Forgot MyChart Username?" link on the MyChart login page. When prompted, enter your name, the last four digits of your Social Security Number, and your date of birth to receive your Username by e-mail.
Sign up for a MyChart account: Or you can ask the receptionist at your next visit to email you a code or go to the Member Services desk and ask to be emailed a code. Or after your visit you will receive an After Visit Summary. The last item on the sheet is the MyChart information with your activation code.

For all other password reset assistance, you must call MyChart Technical Support at 216.444. 1740 or toll-free at 1.866. 915.3383 for assistance. Passwords cannot be reset via email.

Minor proxy access for MyChart allows a parent or guardian to securely communicate on behalf of a patient who is under the age of 18. Access is obtained by completing the Access Authorization with Minor Proxy form. The proxy access becomes more limited on the child’s 13th birthday and is automatically terminated on the child’s 18th birthday.
When you activate your MyChart account, you will immediately be able to send secure messages to your primary care clinician in Internal Medicine, Student Health, and Obstetrics & Gynecology. If you have proxy access for your child, you will be able to message their primary care clinician in Pediatrics.
Maintain physical distance from others. Patients are allowed one companion for regular office visits. Acute Care is open 24 hours/day, 7 days/week, as always, please call first ( 203-432-0123 ).
Everyone entering the Yale Health Center is required to wear a mask, even if you are vaccinated. The Yale Health Center is a healthcare facility and Connecticut requires that masks be worn. Patients entering wearing a cloth mask will be given a disposable face mask.
