At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
The RWJBarnabas Health Patient Portal is a free service that provides convenient access to your personal health care information in a secure online environment. With the patient portal, you can view select test results, access your medical records and track your health information.
Roosevelt General Hospital (RGH) opened at 42121 US Highway 70 during August 2001. The Medical Records Department holds all medical documentation that occurs at RGH and RGH Clinic. Records of admission, emergency room visits, day surgery, X-ray, lab, and more are located at medical records. Per New Mexico State Law, hospitals must retain all records that relate directly to the care and treatment of a patient for 10 years following the patient's last discharge.
The old RGH records are being stored at Plains Regional Medical Center, 2100 N. Martin Luther King Jr. Blvd. Clovis, New Mexico 88101. To find out how to retrieve old patient records that were before August 2001, contact Plains Regional Medical Center at (575) 769-2141, Medical Records Department.
Patients can obtain a copy of their current records from our facility by completing a Release of Medical Records. You must submit a copy of the completed and signed medical records release form along with a copy of your valid picture ID. You can return the form to Medical Record department or fax the documents to (575) 356-7116.

The clinic is open from 8:00 AM to 9:00 p.m. everyday during the week and 10:00 a.m. to 6:00 p.m. on Saturday and Sunday. Even if you don't have a scheduled an appointment, you will be seen.
Emergency situations: If you are calling about an emergency situation please let the phone receptionist know immediately so your call can be handled appropriately.
Clinic services for acute illnesses are avaiable to you after hours and on weekends. And, you don't need an appointment, walk-ins are accepted.

All messages sent to your provider’s office are transferred via a secure server compliant with HIPAA standards.

Due to the sensitive nature of radiology and pathology test results, these are not available on the patient portal and must be obtained through the hospital’s Health Information Department.
RWJBarnabas Health is in the process transitioning all of our hospitals to a centralized patient portal called MyChart where all hospitals’ records will eventually be contained within one system for your convenience. Please be assured that you will always have access to your medical records.
