Current Patients of any of the JCH Medical Group clinics: If you are currently a patient at any of JCH Medical Group, please contact a receptionist to assist you in generating a "JCH Medical Group Portal PIN." Once you have received your pin, click register and follow the instructions.
Here are tutorials on how to login and use the patient portal: Once you register you will have access to your medical records. You will also securely access video visits through the patient portal. You will schedule your video visit the same way you schedule an face-to-face visit: simply call your clinic and ask for a video visit.
Please contact your medical provider’s office for instructions on how to change or reset your patient portal password. Once you can log in to your patient portal, you’ll be able to make changes to your security question or access code.
Not yet a patient of JCH Medical Group: If you are not currently a patient with our office but would like to become one, please contact our one of our clinics to request an appointment.
Family Management allows all members of a family to be managed on one account. To add a member of your family to your Access Pass please click the Family Management link and send the JCH Medical Group the name and date of birth for each family member you wish to add to your Access Pass.
