What to bring to a syringe appointment?

Please bring the following items with you when you come for your appointment: 1 Medications in their original containers 2 Medication list 3 Any test results that are obtained from your referring physician 4 EKG 5 Holter Monitor 6 Stress Test results 7 Letters and past medical histories 8 A list of any allergies to medications 9 Insurance cards & photo ID 10 Referral for treatment from your primary care physician 11 All films (x-ray, CT, MRI, MRA etc.)

How far in advance do you have to call for a reschedule?

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.
