How do I request my medical records through patient gateway?

You will receive notification through Patient Gateway when your provider adopts the new functionality that allows you to request your medical records online. As always, you will continue to be able to request your records by filling out a form provided by your hospital or provider.

How do I enroll in patient gateway?

The easiest way to enroll in Patient Gateway is to request an activation code when you check in at Dana-Farber. We can send you this code by text or email. Once you have the code, you can enter it online and be guided through the enrollment process.
