This interactive web portal empowers all Community Hospital patients and their families to take a more active role in their care by providing easy, secure access to health information and online communication with Patient Portal staff. All of the information in the Patient Portal comes from your Community Hospital Electronic Health Record.
Children 12 and older may set up access to their MyHealth Patient Portal by clicking on the “I do not have an account” button above. To comply with Federal and Colorado State Laws regarding privacy and minors, Community Hospital has chosen to limit access to patient portals for minors over 12 years of age.

Auburn Community Hospital (ACH) HIM Department launched Horizon Patient Folder (HPF) and Electronic Health Record (EHR) system on March 31, 2010, as the legal medical record. HPF stores all patient health information electronically allowing immediate and simultaneous access to the patient’s health information by all caregivers improving patient care for the Auburn and Cayuga County community.
Medical Records. The Health Information Management (HIM) Department is responsible for safeguarding your health information from loss, destruction, tampering and access by unauthorized users. All patients have the right to privacy of health information concerning their care and treatment.
This right is protected by organizational policy, medical record ethics and State and Federal laws. Providing adequate safeguard to prevent unauthorized access to the medical record and to prevent unauthorized release of information contained in the medical record protects patient confidentiality.
Requests for health information or copies of your medical record need to be made in advance by calling the Medical Records Release of Information Desk at (315) 255-7322. Requests for Correction and/or Amendment of your Health Information must be made in writing in the Health Information Department.
Patients are required to sign a Release Of Information (ROI) Authorization form to receive their Health Information at the time the information is received.
