A patient interested in the portal can enroll by clicking “Sign up” on the MyARHChart login screen. Please keep in mind that all 5 fields must match what your healthcare facility has on file: Last Name, First Name, Date of Birth, Social Security, and Email Address.
Connect your account to ARHS. Follow the on-screen prompts to connect your account. You will need to accept our Terms of Service, enter your 4-digit Year of Birth, and accept the release of information. You can connect to the ARHS FollowMyHealth® patient portal if you:
